RN Care Manager/Team Lead - Manages a caseload of complex patients. Provides targeted interventions to avoid hospitalization and ER visits.  Provides care management and care coordination for adults and pediatrics patients with complex illness.  Collaborates with specialists, members of the health care team, and patients/families to ensure the delivery of quality, efficient, and cost effective health care services.  EEO

  • RN licensed in the State of Michigan
  • Three years of experience with adult medicine and pediatric patients in primary, ambulatory care, skilled nursing facility, or hospital medical-surgical setting within the past five years.
  • Prior care management and work experience in a FQHC is a bonus.

Licensed Master of Social Work - LMSW/LLMSW.  Provides direct social work and substance abuse counseling services to individuals and groups including screening, health risk assessment, prevention, individualized treatment planning, patient education, and discharge planning.  The LMSW uses knowledge of health and chronic conditions, evidence-based guidelines, and screening tools to assess over time the mental health, educational and psychosocial needs of the patient/family.  EEO

  • Must attain MCBAP certification within 3 years of employment and complete other required training.
  • Two years of social work experience with adult and pediatric populations in primary care/ambulatory care, home health agency, skilled nursing facility, school environment or similar setting preferred. 
  • Experience with Electronic Health Records a plus.  

Dental Billing Specialist - Assures that billing the system and patient information system operate smoothly and efficiently at our Alger clinic.  Processes and reviews claims and accounts, tracks prior authorizations and answers patient questions regarding statements along with other related job duties.  EEO.

  • Minimum 2 years experience in dental billing & accounts receivable preferred.
  • ICD-10 coding a plus. 

Registered Dental Hygienist - Provide thorough preventative maintenance on all child and adult patients.  Preventative maintenance including prophylaxis, radiographs, complete oral exams, fluoride treatments, and pit and fissure sealants. Provides complete oral hygiene preventative instructions, including periodontal care.  EEO

  • Licensed to practice in the State of Michigan.
  • Two year degree with accredited program preferred. Requires 36 hours of continuing education every 3 years.

Medical Assistant CMA/RMA - Direct patient care given within the outpatient medical office.  Prepares routine patients for care delivered by the medical provider, performs laboratory work, checks in patients, obtains patient history, takes vital signs, EKG's, PFT's, Immunizations, prepares charts using an electronic medical records system and assisting with medical examinations, along with other related job duties.  May require travel to other locations within Arenac, Ogemaw and Iosco counties.  EEO

  • Must be licensed in the State of Michigan
  • Pediatrics & experience working within an outpatient medical office is preferred

LPN - General duties receiving patients and entering data into the computerized EHR system.  Direct patient care given within the outpatient medical office.

  • Current LPN license. 
  • Possesses a thorough understanding of theory and practices in the areas of clinical care typically acquired through extensive practical experience in a professional environment.
  • Two years of recent experience in health care & BLS certified preferred.

RN Staff Educator - Oversight of clinical staff education, and annual competencies, lead orientation, education, and training of clinical and support staff. Supports the Medical Assistant apprenticeship program. 3 years of recent experience in health care preferred.  EEO

  • Current RN license.
  • Possesses a thorough understanding of theory and practices in the areas of clinical care and adult learning, typically acquired through completion of a bachelor’s degree program in nursing and extensive practical experience in a professional environment.
